Referring physician and diagnostic code selection

I have a few questions regarding claims for anesthesia.

1) Do I need a referring physician number for every claim? If so, how do I obtain this? When I search using your search engine the results never include the names of the relevant surgeons / obstetricians.

2) Do I need a diagnostic code for every claim?

Answers

1) You don't need a referring physician for every service code. Our system will warn you if you require one. You can refer to the OHIP Schedule of Benefits for more details.

2) Same as above.
