I was informed by the MoH that we have a set amount of $312 for each patient. What kind of written details are required and how would those be submitted? What would be the next step here?

You can ignore these warnings and submit them with a 0 dollar amount. MoH decides the set amount for each patient according to their formulas, which the information has not been provided to us.
