OHIP error code – V69 – Service date invalid for SLI (Service Location Indicator)
Background
Service date recorded on claim is not valid with the SLI recorded on claim.
Solution
Double check that the correct SLI has been selected for the diagnostic procedure being billed
Double check the service date of the claim
Once the SLI or service date is updated, you can resubmit the claim.
