OHIP error code – V64 - Missing service location indicator (SLI)

The claim is missing the SLI (service location indicator) which is assigned when a hospital diagnostic service is billed by a participating hospital physician/group.

Double check the claim to make sure the correct SLI has been entered. If blank <a href="http://support.dr-bill.ca/en/articles/9237692">choose the correct service location indicator</a> from the dropdown menu, save claim and <a href="http://support.dr-bill.ca/en/articles/9237491">resubmit</a>.
