One of the following codes has been billed for a patient that is not registered on Health Care Connect:
Complex Vulnerable New Patient (Q053A)
HCC Upgrade Patient Status (Q056A)
HCC Greater Than (HCC GT) Three Months (Q057A)
Solution
Contact the Home Health Care Coordinator at your local LHIN to confirm patient is registered on Health Care Connect (HCC) .
