There are three current offices:

Toronto Claims Services Branch
5700 Yonge Street, 5th Floor, M2M 4K5

Hamilton
119 King Street West, 10th Floor, L8P 4Y7

Oshawa
33 King St. West, Unit 403, L1H 1A1

Note: The Sudbury office has been amalgamated to Toronto. You need to change your setting to Toronto if your claims were previously processed in Sudbury.
