00084 – Accompanying patients to a distant hospital where medically required. Billable Per ½ hour. 

MANDATORY E-NOTE:  Must indicate what has happened per each 30 min block or a unit will most likely refuse.

CALL OUT/CCFPP - is payable with this fee code

MOST COMMON REJECTION:   Not enough information has been detailed in the e-note to indicate medical necessity.

TIP : EXAMPLE OF AN EFFECTIVE E-NOTE AND ALL UNITS WERE PAID.  
2130-2200: Continued patient monitoring in transit. No bradycardia noted. BP slightly high. No meds given. 2230-2300: Patient stable. Ongoing airway, BP and heart rate monitoring. 2300-2330: Continued monitoring. No need for intervention or change. 2330-2400: Arrived at St. Paul's Hospital. Transferred to care of ER physician and cardiology team at SPH. Patient in stable condition.
